Demonology Warlock Hidden Artifact appearance is unlocked via the Visage of the First Wakener it's reward for collecting a some heads from Eredar mobs on the Broken Isles or Argus zones.
You need to collect 6 heads include 5 grey-quality item heads and the last head is the artifact appearance.
The items drop in a progressive way using a "flagging" system, meaning you will always get them in order, and once you loot a particular head you will have that particular head "flagged as completed" and be eligible to loot the next head in the order.
