The hard modes involve using the assistance of fewer Keepers. The reward is improved loot and four achievements depending on which difficulty you engage him on.

The achievements are:

• Three Lights in the Darkness (10 player) / Three Lights in the Darkness (25 player) if you use three or fewer Keepers;
• Two Lights in the Darkness (10 player) / Two Lights in the Darkness (25 player) if you use two or fewer Keepers;
• One Light in the Darkness (10 player) / One Light in the Darkness (25 player) if you only one Keeper help;
• Alone in the Darkness (10 player) / Alone in the Darkness (25 player) if you don't accept the help of any Keepers.

To obtain Mimiron's Head mount you will need to kill Yogg-Saron without any of Keepers help.
