The Void-Scarred Windrider drops from the rare elite Void-Scarred Wyvern Matriarch, which spawns inside the Vision of Orgrimmar (Revisited) when you complete a specific sequence of objectives. The mount has a 100% drop chance when the Matriarch is defeated, but spawning her requires a few specific conditions.

To access the Horrific Visions, visit the Horrific Vision Hub in Dornogal. The hub is located just near the Coreway.
To begin, you need at least two masks equipped. The first mask you can unlock is the Faceless Mask of the Long Night. This mask is obtained by completing all 5 objectives in a single Horrific Vision run — meaning you must clear all side zones and the main boss in one visit.
For your second mask, you have two options:
Both options are equally valid — choose whichever side objective you find easier to complete. Once you have unlocked a second mask, you are ready to begin the mount farm.
Once you have two masks equipped and Orgrimmar is the current rotation, enter the Vision and make your way to the Valley of Wisdom. Clear the mobs in this area and complete the zone's objective. After the area is cleared, a pathway to an elevator will open up.

Take the elevator up to the Skyways. Once you arrive, you will see a pack of Void-Scarred Wyverns. You must defeat waves of these wyverns until the Void-Scarred Wyvern Matriarch spawns.
The Matriarch is a rare elite and is tougher than the regular wyverns, but she is manageable with a well-geared character or a group. Once she is defeated, she will drop the Void-Scarred Windrider mount with a 100% drop chance.
