The Tortured Gorger is obtained by completing the Heroic Showdowns meta-achievement and purchasing the mount from Kifaan for 15 Voidlight Marl. The meta-achievement requires you to complete six specific achievements while on Heroic World Tier in the Showdown zones of Naigtal and Val. This is a 100% guaranteed acquisition — there is no RNG involved, no rare spawns to camp, and no low drop rates to overcome. Simply complete the required achievements, earn the currency, and buy the mount.
Access to Naigtal and Val via the portal in Voidstorm rotates on a weekly schedule. Because of this rotation, Heroic Showdowns takes a minimum of 2 weeks to complete. Progress toward earning Heroic Showdowns is account-wide, so you do not need to earn all achievements on the same character.

Before you can begin working on the Heroic Showdowns achievements, you must unlock access to the Naigtal and Val Showdown zones. This is done through a short questline in Silvermoon City.
Pick up the quests Stalkers of the Stars and Veterans of the Great Dark from Riftblade Maella in Silvermoon City. She is located near the Ritual Site and Void Assault vendors.

After completing and turning in both quests, you will receive the quest To the Voidstorm and Beyond! from Riftblade Maella. Continue the questline until the portal to Naigtal and Val is established in Voidstorm: The Howling Ridge.

Since Naigtal and Val rotate weekly, you will complete different parts of the Heroic Showdowns achievement depending on which zone is active. Below is a breakdown of each achievement, what you need to do, and which Showdown zone(s) it can be completed in.
To complete Heroic: Worlds Ahead, you need to complete 15 unique World Quests in Naigtal or Val while on Heroic World Tier.
World Quests in both zones rotate faster than usual, typically lasting about an hour, with new ones appearing roughly every 20 minutes. This makes it easier to complete a large number quickly.
To complete Heroic: Power Creep, you must defeat 10 creatures that have any of the affixes listed in the achievement while on Heroic World Tier.
To complete Heroic: Pain of Command, you must defeat both Naigtal's and Val's specific world bosses while on Heroic World Tier.
