Even though both Heroic and Mythic difficulties work, Heroic is strongly recommended. The reward is identical, and Mythic adds no benefit for this mount.
To begin, travel to Shadowlands using the portal from Stormwind or Orgrimmar.
Once you arrive in Oribos, head to the upper ring and take a flight path to Maldraxxus.
From there, travel to the Plaguefall dungeon entrance:

Before entering, make sure to set the dungeon difficulty to Heroic or Mythic. The mount cannot be obtained on Normal difficulty.
Plaguefall consists of four bosses, all of which must be defeated in a single solo run. At modern levels, this dungeon is extremely easy, and most enemies can be skipped entirely.
Once you defeat the final boss Margrave Stradama, do not leave the dungeon. Instead, turn around and head back toward the third boss area.
Once you reach the area near the third boss, look to the left side of the slime river. There, you will find a friendly NPC Curious Slime Serpent
Approach it and interact. This will trigger a short petting cast. After it completes, you will instantly receive the mount.
Even though the process is simple, missing small details can cause you to fail the unlock.
When farming the Slime Serpent, the main goal is not difficulty — it is speed and efficiency. Since the mount requires a full solo clear of Plaguefall, the faster you move through the dungeon and return to the final interaction point, the better your overall experience.
