Dungeons Difficulty in Diablo 4 is tied with World Tiers. And World Tiers are just difficulty options that tweak how challenging enemies are and determine what kinds of loot and rewards you can get while playing. Diablo 4 has four options: World Tier 1: Adventurer, World Tier 2: Veteran, World Tier 3: Nightmare, and World Tier 4: Torment. Adventurer and Veteran difficulties are available to select from the start of the game. Still, you won't get access to Nightmare or Torment until you finish the Campaign, reach level 50, and start making your way through Capstone Dungeon. Notably, only Adventurer and Veteran are available for the Diablo 4 Server Slam, as it only allows players to play through the Prologue and Act 1 of the Campaign.
In general, World Tiers with more difficult enemies will improve gameplay rewards when selected, including gear drops and both Experience and Gold gains. World Tier 3: Nightmare enables advanced events and activities like Nightmare Dungeons and Helltides. In the table below, we've listed all the differences between all four of the Diablo 4 World Tiers, along with the requirements you need to meet to unlock Nightmare and Torment difficulty.

Doing dungeons, even on Nightmare difficulty, is pretty challenging, and you will need a lot of time to clear enough dungeons to get full high-level gear. There are over 150 Dungeons that are available for repeat play in Diablo IV. Dungeons are instanced content, meaning that once you enter a Dungeon, what you find there is unique to you and separate from any other players, not in your party. No one will stumble along and steal your treasure, but no one will show up to help you. Not only are you in an exclusive "pocket" of the game while you are in a Dungeon but the content within is randomized to a degree - so some of what you find in the Dungeon itself will vary. The lesser enemies and treasures can vary significantly, and there is a 60% chance of encountering an Event in your Dungeon. The Dungeon story, significant encounters, and objectives will remain consistent.
Dungeons vary significantly in length. They can be reasonably concise or sprawl on seemingly forever. Dungeons include a set of objectives that can be completed sequentially and usually culminate in a boss fight against one or multiple bosses. Often certain sections of the Dungeon must be opened by completing objectives before you can continue. Opening a new section will also open new objectives.
Each Dungeon can be completed solo or in a group, as often as you like (except for Campaign and Story Dungeons). However, there are significant rewards for the first time you complete each Dungeon. Your first completion of each Dungeon will usually include special rewards such as a Legendary Aspect and Renown, which increase your power account-wide.
